Collaborative Care refers to the positive interaction of two or more health professionals, who bring their unique skills and knowledge, to assist patients and families with their health decisions.  This model of health care delivery provides better outcomes for patients and improves the overall efficiency of the health care system.

Crossroads Family Practice is very fortunate to have two registered nurses as part of our collaborative team.  Our nurses work closely with your physician to provide quality care for you and your family.
